Who to Refer

We accept referrals for patients requiring specialized cardiometabolic intervention, including those with:

  • Obesity (BMI >30, or >27 with comorbidities)
  • Type 2 Diabetes mellitus
  • Treatment-resistant Hypertension
  • Suspected or confirmed MASLD
Documentation Required

To ensure a comprehensive initial assessment, please include the following with your referral:

  • Recent clinical notes and meds
  • Recent lab results (A1c, Lipids, LFTs)
  • Relevant imaging reports
Process

Physicians can send referrals via fax or secure email. Once received, our clinical team will triage the request and contact the patient directly for scheduling.

What information is required for a complete referral?

A complete referral should include current medications, recent lab results (A1c, lipid panel, LFTs), and a brief medical history focusing on cardiometabolic concerns such as hypertension, obesity, or suspected MASLD.

Is elective imaging optional and how is it paid?

Yes, advanced MRI imaging is entirely elective and optional. It is a private-pay service that provide deeper metabolic insights beyond standard clinical vitals. Costs will be discussed during the initial consultation.
